The washed out look was a separate layer that I placed over the top, in hindsight I don't like it now either
The image was created using a camera on a tripod, a flash gun and a step ladder.
Two photographs are taken, one with the subject balancing on the step ladder and the other (no settings changed on the camera in most cases) taken of the same scene with the step ladder removed from shot.
The two photographs are layered over over each other and the step ladder is simply masked out with the photograph of the empty scene.
As for the snow falling off of the body: I took some extra photographs of snow being thrown into the air using the flash gun and layered those on top using blending modes.
