Should all photos that are framed have a mount? Or is it acceptable to put a photo in a frame directly with no mount?

A matter of personal preference to be honest if they are to hang on your own walls.
If for a customer however, I'd personally always use a mount as it looks more professional and can be used to accentuate the photo in question.
They do look daft round a canvas though, so don't bother there

Mounts are really good at putting a bit of a gap between the glass and the photo, not only does this stop the glass sticking to bits of the photo making it look blotchy but I think it adds depth. Although it does depend very much on the photo and personal preference.
